hostile work environment

I work at a very large company and in my department there is only two perosn; Which is myself and my Manager. When I first started my manager gave me a very poor training, most of the work that I do, I learned it by asking questions to other co-workers. lately she talks to me like I'm stupid, she email explaining things to me like I'm stupid,dum,I don't know anything, She explain to me that the reason that I make mistakes in my letter is because I'm Spanish. She upsetted me so much that I went to HR and told them what is going on, HR schedule her,Vice President,HR, and myself to see what can be done when I left the meeting they favored her on how she runs her department and left me in the cold. I come everyday stress wondering how she is going to treat and speak to me. I have been put on medication because of her. She wrote me up because I don't tell her everything that I am doing in an 8 hour of work.I have to let her know when I go to bathroom, lunch, make copies, and etc...

The best of my story is that she knew that I have some disablity problem and that I can not left up heavy box, and she had to do a project that require for me to left heavy boxes. Know I have injuried my back and she acts like nothing is wrong. Please advise what can I do.............
